Waiver Form:

Participants and Volunteers at each USTSA Sanctioned Event (Educational and Racing) are required to sign a USTSA waiver form.  Participants under the age of 18 years of age must have their form signed by a parent.  These forms must be sent to the USTSA address provided in the Race Organizer Guide within 14 days of the event.
